The Tower Hamlets GP Access Fund service was set up by the Tower Hamlets GP Care group, with the aims of:
- Supporting access to primary care services in the evening and weekends.
- Developing primary care partnerships to ensure maximum value is delivered through the wider primary care community and the primary care estate.
- Taking positive action to address the cycle of demand for primary care services.
- Consultation mode – access to patient registered GP notes. Once in consultation mode please click on All Records on the left hand side and then the consent page will appear asking to obtain consent from patient to access their notes.
- Pathology Request Option. You can do this by using online T-quest service. Please inform patient to contact their registered GP practice for the results.
- Request X-ray and Ultrasound by going to EMIS 'documents template', ‘create letter’ and then searching for ‘diagnostic forms’. Please note Hub GPs cannot request MRIs, Echocardiograms or 24hours Tapes.
- Referrals - Urgent 2 Week Wait Referrals will be coded using the national 2 Week Wait templates also available in the Hub EMIS.Please click here to see the 2 weeks wait referral pathway.
- Coding in EMIS - It is important to ensure accurate coding when entering data on EMIS to make sure practices can import QoF codes reliably and so that Hubs data can be easily audited. Please use holding codes until a diagnosis is confirmed by appropriate investigation, so that we do not get penalised by QOF e.g.’suspected asthma’.
- Templates - All values e.g. BP, Peak Flow, weight, smoking status etc are to be recorded on templates so that they are coded. In Tower Hamlets we use standard CEG templates for all chronic diseases and adding values and data. These can be found by clicking on ‘run template’ in the consultation mode of EMIS web and clicking on the magnifying glass and then typing in CEG, which will bring up a list of the correct templates to use. Click on the appropriate chronic disease for the condition. For example, Hypertension CEG and this will bring up the template and data should be entered into it as appropriate – it will prompt you as to what areas need to be completed.
- Problem list - Please be careful when filing a problem to code it correctly as a new or a first or an ongoing problem (this is particularly important for depression, IHD etc which may require investigations such ECGs) so when filing (F8) please check whether it really is a new problem.
- Chaperone Policy - Each Hub has its own chaperone policy. Please refer to that or ask member of staff working in the hub for more information. Click here to see THGPCG Chaperone Policy.
- Safeguarding Children and Adult - Each hub has an available algorithm indicating the procedure for managing safeguarding concerns in an out of hours/extended hours setting. All safeguarding concerns must also be highlighted to the patient’s own registered practice via the hub NHS.NET email by the hub GP. - Complaints and incident policy - Complaints should be made in writing to the hub practice indicating that it is related to the hub service and will be dealt with as per the TH GPCG complaints policy – via the GPCG complaints procedure with response in the recommended time frames. - Health and safety policy - within each hub practice there will be the contacts for Health and Safety concerns out of hours. All Health and Safety concerns must also be highlighted to the registered practice via the hub NHS email by the receptionist on duty.
